Evidence you decided on before you started
The defining move is committing in advance to what would count as evidence. If you decide afterwards that engagement improved because the room felt different, you have written a reflection. If you decided beforehand to track on-task behavior at fixed intervals, you have data, and the finding can contradict you.
The cycle structure matters too. Plan, act, observe, reflect, and then genuinely plan again on the basis of what you observed. Many write-ups describe one intervention and stop, when the assignment expects at least one full cycle feeding into the next.
The third piece is positionality. You are the teacher and the researcher, studying students you grade. That dual role is not disqualifying, it is the method, and it has to be addressed directly rather than left implicit.

Your program decides, and one complete cycle with the next plan visible is a common minimum. What matters is that the second plan is genuinely shaped by what the first cycle showed, rather than being what you intended to do anyway.
Whatever you decided in advance would answer your question and can be collected while teaching. Work samples, structured observation, exit tickets, attendance, short surveys, assessment scores. Feasibility matters as much as rigor here, because evidence you cannot actually collect mid-lesson does not get collected.
Usually yes, from your institution and often from your school or district, and sometimes parental consent depending on the students' age and what you collect. Ask early. This is the most common reason an otherwise good action research project has to be redesigned.
That is a legitimate and often more interesting finding. Report it, and use the observation stage to say why, then show what that implies for the next cycle. Write-ups that conclude everything improved with thin evidence score worse than honest null findings.
