A common structure is three pages on Vygotsky followed by two paragraphs saying the teacher should scaffold. That is the theory and a conclusion with no reasoning in between. Application means using the theory's own concepts to explain what is happening for this student, and deriving the response from that explanation.
The second issue is theory selection. Many assignments give you a case and expect you to choose a framework. Picking one that does not fit the phenomenon, usually the one most recently covered, produces strained analysis. Motivation problems and cognitive load problems look similar from the outside and need different theories.
The third is overclaiming from development stages. Stage theories describe typical patterns, not rules about individuals, and writing that a nine-year-old cannot do something because of a stage is the kind of claim graders flag.
