A report that lists scores test by test has not integrated anything
The common weak structure runs through each instrument in turn, reporting its findings. The reader is left to assemble the picture. An integrated report organizes around domains or around the referral question, drawing on whichever tests bear on each, and says what the convergences and the contradictions mean.
The referral question is the discipline. Every section should be earning its place by answering what was asked. Reports that include everything administered, regardless of relevance, are longer and less useful, and that is precisely what supervisors penalize.
Then recommendations, which should be specific enough to act on and addressed to whoever has to act. A recommendation that the client would benefit from support is not actionable; naming the type, the setting and the frequency is.
