A student who records an exchange where they gave premature advice, identifies it, names why it was unhelpful, and says what they would do instead has demonstrated exactly the skill the assignment is teaching. A student who presents a smooth encounter has not, and experienced instructors recognize the difference quickly.
The feelings column is the part most often left blank or neutral. Discomfort, frustration, feeling out of your depth: all normal in a practicum, all relevant to how you responded, and naming them is part of what is being assessed rather than an admission of weakness.
Assessment writing is the other major genre. Strengths-based framing is expected in most programs, which does not mean ignoring risk. It means describing what the client and their environment bring alongside what the difficulties are, rather than producing a deficit list.

Usually yes, and it typically scores better. The assessed skill is recognizing what was unhelpful and knowing what to do instead. A flawless session gives you nothing to analyze and instructors are generally sceptical of them.
What you actually felt, including discomfort, irritation or anxiety. It is not assessed on whether the feelings were appropriate. Leaving it blank or neutral is the most common way to lose points on an otherwise sound recording.
Describing what the client, their family and their environment bring, alongside the difficulties, and building the plan from both. It does not mean minimizing risk. An assessment that omits genuine safety concerns to sound positive is a serious error.
Enough to explain your choices. Most programs want you to name the practice model or theory informing what you did. Naming it without connecting it to a specific moment in the encounter is the usual half-credit version.
